Question

Will my insurance pay for my visit and treatment? What insurances do you accept?

Answer
We are an out-of-network provider for all insurance companies. Depending on your insurance and the coverage it provides, we estimate your portion and that portion is expected at the time of service. We will bill your insurance company or provide you with a completed insurance claim form called HCFA-1500 that you can submit to your insurance company. Many (but by no means all) health insurance plans provide some degree of coverage for “out-of-plan” medical care.

You may want to call your insurance company and ask the following questions regarding your coverage:

  • Do I have out-of-network benefits?
  • If so, how much is deductible?
  • How much of out-of-network deductible have I met for this year?
  • How much do I have to pay out-of-pocket before insurance covers 100% of allowable charges?

Do I Need a Referral?

Answer
No, you do not need a referral if you have insurance plan that allows you to go to an out-of-network provider. Your services will be covered as outlined in your insurance benefits booklet.

What is the fee for a visit? How much do I have to pay? What other costs are there?

Answer
The fee for your initial visit is $350. It includes initial history, a physical examination, and review of medical records if any, and based upon this data, Dr. Sultan will determine the probable causes for your symptoms and begin a treatment plan. There are additional fees for lab work, testing, and any other treatment services provided during this appointment. These can vary greatly. All fees will be explained and reviewed with you prior to doing any testing or treatment. After your insurance benefits are verified, the office staff determines what your estimated portion is. This portion is payable at time of service. After your insurance company has made actual payments we will bill you for the remaining balance if any. If your insurance company has not paid in 60 (sixty) days from the date of submission then it becomes your responsibility to make due payments during that time.


Question
How long should I expect my visits to be?

Answer
First office visit generally takes two to three hours. You will be with Dr. Sultan for one hour and then allow one to two additional hours for possible testing and treatments.

What is the age range of patients you see?

Answer:
We accept patients of all ages-infants, children, adolescents and adults. Regardless of the age, we are here to find the cause of the illness and provide the answers to better health.

I have no insurance; do you accept patients with no insurance?

Answer:
Certainly we do, and we do welcome them.

Payment is expected at time of service.
